San Joaquin County Health Care Services touches lives in every sector of the community. Creating a sense of community is at the heart of it all and partnerships are what promotes health access in San Joaquin County.

 Health Plan of San Joaquin
Health Plan of San JoaquinThe Health Plan of San Joaquin is the community's local leader in providing low-cost health insurance to nearly 60,000 members in San Joaquin County. It is the largest provider of Healthy Families insurance for children as well as Medi-Cal enrollment. In addition, the Health Plan of San Joaquin now administers the health care program for over 7,500 San Joaquin County employees and their families.
 Healthy Families
Healthy FamiliesWorking to solve the problem of uninsured children in San Joaquin County, Health Care Services, Concilio and Community Medical Centers have signed up thousands of families for the state's Health Family Program. Children get access to a full range of health services including immunizations, doctor visits, vision and dental care, hospital care and prescription drugs.
 Thornton Health Fair
Migrant farm camps have often been overlooked as sites for health clinics. But, with Community Medical Centers, Concilio, Health Plan of San Joaquin and San Joaquin County Health Care Services, children are assured immunizations, and families have access to a doctor.
 Taylor Elementary School
Health Care Services teams up with San Joaquin A+ and community leaders to promote better health. At Taylor Elementary School in south Stockton, children and their families learn about health promotion through the Taylor Healthy Start Program. Thanks to a partnership with the Housing Authority, children and their families can visit the nearby Conway Homes clinic for convenient doctor's visits right in their own neighborhood.
